IMPORTANT INFORMATION PLEASE READ: The following pages will ask you for your name and address for shipping. We do not ship but we do need this information so please fill it out. While filling this out remember that your name and address MUST match your address on your credit card or else it will get rejected. This rejection sometimes will look like the funds are gone because some banks put a hold on funds for a day or two and then release it. This is the bank that dose this and not us so please make sure those addresses match.
Expired Cards should be registering for a PALS Reentry class and not a PALS Recertification class.
Non American Heart card holders should be registering as a PALS Reentry class and not a PALS Recertification class.
